Case Managers and release planners  

Please have your client sign a Release of Information (ROI), complete the RCCS Screening Sheet.  When completed, please scan the ROI, RCCS Screening Sheet, PRT Packet, & HRU note (if applicable) and email to RCCS, or fax to 612-767-9832 (Minneapolis) or 651-489-0184 (St. Paul).


Supervised Release Agents

Please ask your client to call 612-767-6601 to schedule a Rule 25. If your client is in a county jail, please contact RCCS at 612-767-6601.
